Expanding Access, Expanding Hope

For more than a decade, Springbrook has led efforts to provide coordinated, person-centered care for people with intellectual and developmental disabilities (I/DD). We have seen firsthand that when healthcare is consistent, compassionate, and designed to meet people where they are, lives are changed—individuals are healthier, families gain relief and trust, and communities grow stronger.

Since opening the Springbrook Wellness Clinic in 2019, Springbrook has partnered with Bassett Healthcare to ensure patients receive coordinated primary care from trusted, consistent providers supported by integrated electronic health records. This work builds on a strong foundation, including the launch of a school-based health center at the Springbrook School that has reduced hospitalizations and emergency room visits among students. These enduring partnerships show that Springbrook is not testing a new idea—we are expanding a model that works.

Expanding the Springbrook Wellness Clinic will allow more children, adults, and families to receive care from familiar providers in a setting designed for people with I/DD while also serving the broader community. Our experience shows that specialized primary care—across a lifetime—makes a measurable difference.
